If you would like a hard copy pad of referral forms, please contact us and we'll deliver some to you. Don't forget you can send referrals easily and efficiently using our e-Referral system!
Access to Reports and Images (PACS)
Our images are stored securely in Centralpacs. If you would like access to view images and reports for your patients, please visit www.centralpacs.co.nz. and follow the instructions to request an account or to log in.
If you just want a report we can promptly send these directly to you via EDI, email or fax. Please email [email protected].
