2. Online Appointment Booking

• Submit the booking request form with your details, scan requirements, and any referral documents (maximum 2MB; accepted formats: PDF, DOC, DOCX, ODT, RTF, or images).
• Our staff will contact you via your preferred method (phone or email) to confirm, discuss, or schedule the appointment.
• Appointments must be attended in person at our clinic location. Please arrive 5 minutes early.

Certain services, such as maternity or ACC-funded scans, require a referral from a medical practitioner. Without a valid referral, you may be responsible for the full cost of the service.

Cancellation Policy: You must cancel or reschedule appointments at least 4 hours in advance by contacting us at (07) 929 4151 or email [email protected]. Late cancellations or no-shows may incur a fee equivalent to the full service cost. We reserve the right to charge for missed appointments to cover operational costs.

We make every effort to accommodate your requested time, but availability is not guaranteed until confirmed by our staff. Technical issues with the booking form do not constitute a confirmed appointment.

3. E-Referrals for Medical Practitioners

The Website provides an e-referral system exclusively for qualified medical practitioners to submit patient referrals electronically in a secure, paperless manner (accessible via https://theultrasoundclinic.co.nz/referrer-information/e-referrals/).

• The e-referral feature is strictly for use by medical professionals. Patients must use the patient booking form and may not submit referrals on their own behalf.
• To submit an e-referral, complete the online form with all required fields (marked with an asterisk), including patient information, examination details, referrer information, signature, and preferences.
• Logging in or registering as a referrer simplifies the process by pre-filling your details.
• Alternatively, referrers may download and use our digital PDF referral form or request hard copy pads.
• All referrals must include accurate and complete information. We reserve the right to request additional details or clarification.
• Submitted referrals are processed securely, and you will receive confirmation as appropriate.

• Access to patient reports and images may be available through systems such as Centralpacs, with reports sent via EDI, email, or fax upon request.

4. Purchases and Payments

Purchases on the Website are processed securely via Stripe checkout. Currently, purchases are limited to gift vouchers for non-medical 4D Bonding Ultrasound and Early Gender Reveal Ultrasound services.

• All prices are listed in New Zealand Dollars (NZD) and are inclusive of GST where applicable.
• Payment must be made in full at the time of purchase using a valid credit or debit card accepted by Stripe.
• Upon successful payment, you will receive a confirmation email with your voucher code.
• We reserve the right to refuse or cancel any order for any reason, including errors in pricing or availability.

Refunds and Exchanges: All purchases are final and non-refundable, except where required by New Zealand law (e.g., under the Consumer Guarantees Act 1993). 5. Gift Vouchers

Gift vouchers purchased on the Website are subject to the following terms, in addition to any service-specific conditions:

• Vouchers are valid for one examination only (either 4D Pregnancy Bonding Ultrasound or Early Gender Reveal Ultrasound, as specified at purchase).
• Appointments must be booked in advance via our booking form or by phone/email, quoting the voucher code.
• Present the voucher code when booking and bring a printed or digital copy to the appointment.
• Vouchers cannot be exchanged for cash, transferred to another person, or combined with other offers unless specified.
• Valid identification may be required at the appointment to verify the voucher holder.
• The Clinic reserves the right to modify these voucher terms at any time, with notice provided via the Website.
• The terms and conditions for the selected service (as outlined below) also apply to voucher redemptions.
• Vouchers expire 12 months from the date of purchase unless otherwise stated.

6. Service-Specific Terms

Our services are provided by qualified sonographers using advanced equipment, but all non-medical scans (such as 4D Bonding and Early Gender Reveal) are for entertainment and bonding purposes only and do not constitute medical advice or diagnosis. Always consult your healthcare provider for medical concerns.

6.1 4D Pregnancy Bonding Ultrasound

• Recommended between 29 and 32 weeks gestation for optimal views.
• Scan duration: 20-30 minutes if combined with a 3rd Trimester Growth Scan; approximately 20 minutes standalone.
• Includes real-time 3D viewing and images via our My Scans app (no additional charge).
• A prior 20-week anatomy scan for the current pregnancy is required for standalone 4D scans.
• No referral is needed for the 4D scan itself, but one is required if combined with a funded growth scan.
• Clear facial views are not guaranteed due to factors like baby's position, umbilical cord, hands, amniotic fluid levels, or maternal tissue. Staff may suggest a short walk to encourage movement, but no refunds will be issued for suboptimal views.
• Payment covers the bonding experience for you and any accompanying family/friends, not solely the images provided.

6.2 Early Gender Reveal Ultrasound

• Available from 15 weeks gestation (16 weeks recommended if elevated BMI).
• External scan preferred; internal scan may be discussed and performed on-site if needed for clarity.
• Includes gender determination and an optional surprise envelope.
• A prior 12-week Nuchal Translucency (NT) scan is required.
• Scans will not be performed before 15 weeks.
• Gender determination is not guaranteed in all cases (e.g., due to BMI or baby position), though rare. No refunds for inconclusive results.
• Payment covers the experience for you and any accompanying family/friends, not solely the outcome or images.
